Directions to Our Facility

From Dallas - Travel east on Washington past the hospital and across the railroad tracks to Monmouth Cutoff. Turn left onto Monmouth Cutoff and travel east approximately 3 miles to The Shaggy Dog. We are located on the left hand side (North side) of the road.

From Salem - Travel west on Highway 22 to Highway 99W, and take the "McMinnville/Monmouth" exit. At the stop sign, turn right onto 99W and travel south 2 1/2 miles (through Rickreall and past the Polk County fairgrounds and museum) to Clow Corner Rd (double flashing yellow light). Turn right and travel west 2 ½ miles on Clow Corner Rd. We are located on the right hand side (North side) of the road.

From the Lincoln City - Travel east on Highway 18, take the Highway 22 (Dallas/Salem) exit and continue east until you reach the Highway 99W exit. At the stop sign at the top of the overpass, turn right onto 99W and travel south 2 1/2 miles (through Rickreall and past the Polk County fairgrounds and museum) to Clow Corner Rd. Turn right and travel west 2 ½ miles on Clow Corner Rd. We are located on the right hand side (North side) of the road.

From Corvallis - Travel approximately 25 miles north on Highway 99W to Clow Corner Rd (double flashing yellow light). Clow Corner Rd is approximately 3 miles north of the intersection of Main and 99W in Monmouth.  Turn left  and travel west 2 ½ miles on Clow Corner. We are located on the right hand side (North side) of the road.
